Data Entry Web Portal - SQL Server/ASP.NET

The project consists of creating a data entry web portal to allow end users to enter data collected on paper forms into web forms. Once input, the records must be viewable online as well as through five seperate reports using SQL reporting services. A preliminary database diagram and web form wire frames are availalbe for your review. Requirements: The captured data must be stored in a SQL server database. Once data has been captured using the web forms each record should be viewable by the end user. Five custom reports will be required with the ability to export in PDF or Excel forma using SQL Reporting Services. Deliverables: Deliverables will include the SQL database and the ASP.NET web portal code. Timeline: Employer wishes this project to be complete within 30 days of acceptance of bid.
