Book Cover Design Including Cover Illustration for Biblical Fiction Book

I need a cover design for The Lamp of Darkness, the first book in The Age of Prophecy Series. The book is a work of Biblical Fiction, set in the times of Elijah and Ahab. I myself am a Rabbi living in Israel, and this book, while fictional, is strongly rooted in the Jewish tradition. I believe that readers certainly do judge books by their covers, so I want a fantastic cover design. I had been working with one designer for the past several months, but parted ways because he could not either 1) find existing royalty free images that reflected the main ideas of the book, or 2) illustrate them himself. Having gone through this process, I believe it unlikely that the final cover can be made exclusively using existing images and will probably require some illustration elements. As discussed in the attached document, which gives specific examples of what I'm looking for and elements that the book contains, I like book covers that have a single element that draws the eye, such as Catching Fire or the original Twilight covers. I'd also like something that captures the period and feeling of the time, firmly creating the Biblical setting. Because I had spent so long working the the first designer, I'm looking for a fairly quick job. I want to be well along the process within a week. Also, I'll want several versions, with different size covers for the physical book, ebook, and audio books, so please include all of that in your bid. I also want the designer to prepare the cover for publication, which means being able to put it onto the templates of Lightning Source, etc. (which should be pretty basic for any cover designer). I have attached a document with specific cover tastes as well as a plot summary and list of elements that could work well in a cover. Please attach examples of covers you've done in the past. If you have specific ideas to share about how you would do my cover, I'm all ears.
