I want to be able to send a web link to a recipient (e.g. via email) that once clicked opens up a web page and prompts the recipient to add their name, email, company, job title etc. They would also have to click a box that says "I agree to the terms and conditions". There would be a link to the online terms next to the check box.
Once the user clicks "agree", this would unlock the document (a PDF, PPT or Word Doc).
Unlocking the document via the form would send an email to the recipient alerting them that they had unlocked the document and containing an attached copy of my terms and conditions. An email would also be sent to me directly alerting me that the recipient had unlocked the file and containing the relevant details they had added to the form. Both emails would detail the recipients IP address in the body of the automated email. All fields on the form would be required fields.
From a cosmetic point of view the form would contain my company logo, title, link to my website etc.
