I have a full working working Adobe file with forms. I need several sections to be required based on conditions. You will need the latest full version of Adobe to create these conditions. 1. Spouse information needed if individual answers they are married 2. Medical History under Section C-1 * If user answers No, then the medical conditions are not clickable below * If user answers Yes, then user can click any of the conditions and must click at least one. 3. Section C #4 (Pregnancy) If user answers "yes" then user must answer 4a, 4b, 4c and 4d. Under Section D *If user clicks any except "none", they must fill the first row *If user clicks Spouse on any answer, they must fill the second row in addition to Employee (first row) *If user clicks children, then they must enter row 3 along with 2nd and/or 1st row. Attached is the workable PDF file and a scanned version with the above notes
