The function of this simple form and script would be to do the following:
Allow a person to record the project ID, priority, supplier details and item details.
The current google form and script extracts the "username" email address of the google user because we are using Google Apps.
Not sure if this can be done with a custom form... If not just a field to for the requesting person to enter their email would be ok.
A formatted approval request email would be sent to an email address associated with a project ID
If there is a 2nd approval person associated with the Project ID, an escalation button is included in the approval request email as an option.
When Approved/Escalated/Rejected Google sheet is updated with status label & email address of the person how actioned.
Emails also sent to the original persons email address who submitted request updating the status of order.
Some of the script has already been written, this may or may not be of help.

Attached is a concept for the form based on a 2 step entry, If the form can be a single step and the data stored into the google sheet that would be even better.
