Custom form app

Looking for an app that will allow our field tech to submit inspection reports from tablet or phone. about a 10-20 fields, some text fields, dropdowns, yes/no, checkboxes, notes, would like to capture time/date and GPS location, and last is to be able to while in form is to take a photo from onboard cam, insert into the form, maybe on second page and be able to markup up with basic tools (pencil tool mostly to add quick notes/dimensions or drawings). I have done this using free form builder tools but the last part with camera, inserting photo to form, so it it can be marked up has puzzled me. I have sample for you to see / try. final form would be emailed and or saved to web site for review. if possible, when tech submits form from the app, if no data connection is available, app will hold form/data in a queue and submit when data connection becomes available. if this works out, we will want more forms added to app (multiple time sheets, field survey/reports, inspections....)
