I need a from, a form with AJAX.
I provided a sample page of what the fields should be.
This information is then inserted into a database.
Then, I need this stuff to be able to be edited.
BUT, I need an "Add New" option in the drop-downs for the edit page.
When a person selects the "Add New" option from the drop down, another box should show up allowing the user to add a new option. Once they submit that form, the field is populated with the new item, and the drop-down menu needs to be re-populated with the new item the user just submitted.
I need this done now. Now later than 5:00pm EST USA
