Classic ASP/HTML Registration Form (7455)(repost)

We have a Classic ASP page with a form that needs to have fields added that should display only if certain selections are made on an existing select box. If anything other than "Physician" is selected in a select box named providerID, the fields below should display. If Physician is selected, these fields should not display. Source files will be provided as well as a link to functional sample form on a web server (but without the fields below). The new fields must display based on making a selection in the select box, not based on adding a submission button. In your bid, please indicate the time frame you estimate needing to complete the work.
