Creating a Tool for an online database that allows for easy data entry and viewing

I'm looking for a very simple tool that I can put on my website that will allow for various employees to easily add/view/delete/edit information to/from the online database. The current database is written in MySQL (it was a custom job), and we have elements of the tool already coded in HTML, PHP, and CSS. The website is running on Joomla, hosted by Go Daddy.
The flowchart for the project can be seen in the "Control Center Flowchart Private" pdf. This flowchart gives a good indication as to the number of different elements in the database, and some of the different fields that the tool consists of. The database architecture can be seen in the "ERD private" pdf, demonstrating the approximate number of fields and keys. I have also provided you with a brief example of what the current tool looks like. For the time being, this tool will be only for internal use, but eventually we plan on making various aspects of the tool available to our external customers, and we would prefer to use the same freelancer who designed it in the first place for that project as well.
Essentially, we're looking for someone who can 1) create the second half of the tool, the view/edit wizard, which would provide us with the ability to read from the database and display the information in a more appropriate format (almost excel-like, see the ViewEditLayoutV1Private pdf) and 2) Connect the tool that we have created to the database.
