Access Database Software

We require a small Microsoft Access database to be developed to facilitate data entry of basic patient data and two set forms. Additional requirements include the export (CSV format) of entered data for later analysis and validation of one field (code + examples provided). Update: 1) Have uploaded new ZIP file with basic specifications of the fields 2) I must stress that the user of the program will be inputting a lot of the data at once. User must be able to add new patient, and then easily enter data for both forms without leaving keyboard. (Using "Tab" key etc) 3) I will likely select a bid within the next 2 days. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) All work to be completed as per attached document 4) Final database should be able to be used easily and efficiently for data entry of the provided forms 5) All work to be tested before payment made 6) Deliverables must be in ready-to-run condition, as follows: a) Access Database that works on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law.
