Create a Single Page Like This with slot for Video

I'm looking for someone to create a simple page "JUST LIKE THIS ONE [login to view URL] For us. We'll supply you with the video as well as the code to use for the optin form. This is a VERY EASY JOB "IF" you know what you are doing so bid fairly and low.... I prefer to have this set up as a WordPress Blog if possible, but am open to other options, let me know what you can do. The page must look the same.
