Create wordpress contact us plugin

I would like someone to build me a plugin for wordpress that creates a a contact us page. It need to a couple of spefic things. 1. Have a image verfication to prevent spam. 2. When filled out the form would be emailed. 3. Have a subject box which when selected can determine what email address the response is sent to (all tailorable in admin) 4. The ability to add extra fields (including drop downs) which are tailorable on admin. 5. The ability to alter the tailor the top section of the contact us and a thankyou page. Please send me example of pervious work and tell me a bit about yourself. I'm looking for someone who is avilable online, have very good english and has a good history. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables): a)?
