Customised registration pages, SMS and payment gateway

We urgently require registration pages and forms to be created for our website and on our server. I need a design custom-tailored to my specifications, written in PHP, MySQL, AJAX, and proper JSP coding All data must be stored in our MySql database which you will also need to create. You must also download and customise ModX CMS for me please. The forms have approximately: 35 x short / long open text fields 10 x drop downs 10 radio buttons Some questions have on-click responses. We also require a payment gateway (PayPal) and SMS gateway (Esendex). We would prefer to work with someone in Sydney time zone if possible.
