change an exzisting webform to a 3 page one(no database needed):
first page:
the first page of the form will remain the same, see : [url removed, login to view]
Second page:
the second page of the form must have the following 3 field's :
field 1 - In which area of "------" would like to rent?*required
(for example : bellville)
field 2 - please enter your name *required
field 3 - preferred way of being contacted?*required
(enter your email or telephone number)
submit
use a php script to email the form
Third page :
thanks "-----" a rental agent from "------","-------" will contact you regarding rental property !
Adsense ad below the above mentioned text.(create a php file just for the adsense code please)
I want the look,style and fonts of the second and last page of the form to look like this([url removed, login to view]) see attached gif file

the project cost will be split up in 2 milestone payments - first payment will be made when i see a working demo of the script and the second one will be made after the script is uploaded to my server. I provide the current forms files if needed.
