Basic PHP/MySQL web app

Our requirement is for a web-based referral application that allows users to create a password protected account, submit details of referrals and monitor referrals. I have created a detailed spec (attached) with the following: - Textual specification for the task - Wireframes - Information Flow The code must be written with a clean and easy to maintain modular approach. All functions, variables etc must be clearly documented and viewable via phpxref. I have attached an example php class ([login to view URL]) written in this manner. I do not require any CSS styling, only the basic html. ----------------------------------------------------- As a separate requirement (but conditional on completing the referral system) we are looking be for a team of developers for a long-term association to do the following: - Maintain a dedicated server (Fedora) - Maintain and develop an existing php/mysql web-app Our requirement will be for at least 20 hours work a month. The group of developers must speak good english and have the following skill set: - Linux fedora OS - php4 - javascript - mysql - xml, xsl-fo, fop - object-oriented programming Experience with the following would be welcome: - google mapping As an example our next requirement is: - Adding a google mapping module to an existing web application. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
