Change Simple Html To Match Model Page

This is a really fast and easy job for an experienced html coder. On a webpage, I have a video with an opt-in box next to it. <[login to view URL]> I want the page to look like the below page. [[login to view URL]][1] After you have the first page looking like the model and still working on my webiste and I've approved it Then I have another video page (different video) that should have the exact same setup (actually there's 4 pages but they have the exact same content - I use the different pages to count user selections). You're job is to get them working on my website. That's it. You'll provide simple html and graphics if needed that resembles the other page and has the video still working. One more thing. When someone opts-in to the model page (worldwidebrands) then a popup appears but they don't leave the page. If you know how to do that then please say that you can in your bid.
