SKILLS: WaveMaker, PowerBuilder-web, PHP, Joomla, mySQL 1. We are developing a Joomla CMS website on an internal Windows 2008 Server/IIS. This website will be assessable from the Internet as well as the Intranet. 2. We need a C.R.U.D. application with login/password security. This application (hosted on the Windows server) will save its information to a mySQL database. Once saved, we will be able to retrieve the information back to the screen into either of the two forms to be created by you, the developer. (We will give you all the specifications for these forms.) Example of the first form: 1. There are 140 questions. 2. 40 fields, if answered incorrectly, will trigger an exception pop-up box. 3. This pop-up box will read the violation from a violations table, allowing override, and provide a comment field. 4. If over-ridden, this override will be written to a separate table for later export to .CSV file. 3. Since these forms may stay opened for hours before completion, we need a way to save periodically while maintaining a lock on the form so that no one else can edit it. 4. We will need a menu option on each form to be able to export data (by date range) to a .CSV file. Attached is an example of the top portion of the first form.

Dear Texasgirl, personally have years experience in huge forms (250+ fields) for application forms used with score-card & rules-based systems, for local companies ITC (TransUnion credit bureau) and several of the banks (e.g. Credit Loans, Home-loans and Wills client application forms). The forms are completely defined in data tables (no code), containing the categories of questions/answers (various types) and their validation rules - criteria/exceptions or violations. The front/back end routines render the capture forms from the defined data (model). It's basically a general template and can be re-used for as many forms or applications as needed. The forms are usually shown as tabs or sections to make it manageable for the large number of fields. The defined data model (and rendering/validation code) can be extended for future/other requirements/uses as needed. AJAX stores the answers real-time + validates so violations are actioned real-time providing the user with additional resolution options [can be in a popup]. Forms can remain open/locked to a user capturing as long as required until form is completed/submitted. Data/answers (including violations) can be extracted based on criteria, e.g. date range into various formats (.csv/.xls etc) for further processing. I hope this provides some insight into successful large form capture, hacking away at the front-end and data-storage usually is time-consuming and does not solve these requirements in the long run, specially when [future] maintenance and changes are introduced. We've had good results following the explained approach. The project expectations has been outlined and described well, good job.
