Amazon S3 Upload Interface for HTML

This should be pretty simple: I need HTML script that I can insert into my own webpage that will give users the ability to upload ANY type of file to a folder on my Amazon S3 account. I need to able to insert my access key, etc, on my own, and if need be, change the bucket or folder destination. (I know enough to be able to do this). What I need you to do is provide to me a basic interface, presumably with a "choose file" and "submit" button, then a place to insert a forwarding page address when the file is successfully uploaded.? This doesn't have to be gorgeous, and I also don't care if you provide to me the same script you may have written for someone else, assuming you're allowed to do that.? I'm looking to move on this immediately.
