Comment/Complaint Section

Need a php - mysql database to be able to record/view/search commments and complainst posted to the site by end users. Categories for submittal are as follows: First Layer Second Layer Third Layer Requests New service Current service change Install bench Install shelter Other Compliment Driver Bus service Other Complaint Bus early List all routes individually Bus late List all routes individually Missed transfers List all routes individually Discourtesy Careless driving Vehicle Other Other The following note would be good to have on the form to help the fill it out. “Where applicable provide as much information as possible such as bus number, time of day, date and location. This will assist us in researching your request. Thanks?? Also if possible I would like to have a check box for them to check if they wanted to be contacted and if checked it would send an e-mail to a specified address so that we knew they wanted a response.
