Create a daily feeding schedule for baby

Need a program in PHP to track daily feeding sessions for a baby. User inputs 5 pieces of information: 1) Birth date of baby 2) How long on left side (in minutes) 3) How long on right side (in minutes) 4) How many ounces of breastmilk from a bottle 5) How many ounces of formula The program would show a table showing the current day and age (in months, weeks and days) of baby as a heading and then 6 table headings displayed in columns: Column 1 - keep a running count of the number of "Sessions" per day Column 2 - record the "Time" of each feeding session (defaults to time that data is entered) Column 3 - "Left Side" (in minutes) Column 4 - "Right Side" (in minutes) Column 5 - "Bottle of Breastmilk" (in ounces) Column 6 - "Bottle of Formula" (in ounces) a) The table would show running totals at the bottom as well as yesterday's totals and the average for the last 7 days. b) User would need to be able to edit all past information c) User would need to be able to see/scroll through data from past days d) Need to create option to show "full history report" - User must be able to see a list of all of the days in which data is available that shows the date (day, mm/dd/yy), the total number of sessions, and the totals for Columns 3-6. e) PHP code must be commented very clearly and extensively for future reference
