Add ons to PHP Site - RELISTED

I've been let down on this one, so if you previously bid, please do so again - I now need this done very urgently. I have a customer with the php classifieds turnkey website which I set up for him. He is working with several High Schools using the website as a fundraising tool, whereby standard advertisers will choose the school they wish to support and that school will receive a 'commission' payment at the end of the month which will be 50% of the advertising cost. Somewhere in the Registration screen, he needs the new member to choose which athletic program he/she wishes to support with the placement of their ad from a drop down box. He also needs to be able to add to the list from the admin section of the website. He requires the database to somehow be able to keep up with this function so he can print off monthly reports for each High School Program which could be sent along with their check.
