A QR Barcode Generator for our website.

Hi We have a website [login to view URL] and we require some code written to add to our website offering something similar to what is on [login to view URL] but with slightly different options. So basically some one can fill in a field with either 1. a URL 2. a Facebook or Twitter account 3. an email address 4. Phone number including international code etc. There would need to be someway of identifying which they want, so clicking the option tab like with the [login to view URL] site above or a radio button like on [login to view URL] I'm open to offers on this. and the code YOU write will create a QR code say 500px x 500px and show this in an area underneath or to the right. They can then choose to download this or email is to themselves, and then we can retain their contact info. Hope that makes sense, I presume its a pretty standard thing these day, I guess its a PHP solutions but I will leave this to you. The website is a Wordpress site so the code needs to be compatible.
