Disclaimer with log-off

I am looking for a simple program that pops up when a user logs in to the network. It basically has a disclaimer saying that the company network can only be used for business purposes (blah, blah, blah...) and then 2 buttons "I Agree" and "I Disagree",Clicking "I Agree" will allow the user to continue on, clicking "I Disagree" will log the user off. There should be no way around the application (ie- Alt-F4, Alt-Esc, Alt- tab, Ctrl-alt-del, whatever...) ## Deliverables Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. Complete copyrights to all work purchased. ## Platform Should be able to run on Win 9x machines, as well as W2k machines.
