Build a website that allows users to create a profile and ask specific questions

I need a developer to build a website that allows users to create a profile and ask specific questions of the user. I want to save the profiles and ask certain questions. I want the user profiles to be stored so that I can access them. I want to be able to access this code later to have someone do work on the website. The website should be fairly simple and clean looking. (enter Serial number or decline) Can you please answer some questions about lawn? Does your lawn have any issues with bugs or weeds? (Yes/No/Not sure) soil type (Clay soil/Silty soil/Sandy soil/Loamy soil/Not sure) grass type (Tall Fescue/St. Augustinegrass / Floratam/Bermuda/Carpetgrass/Bermuda/Zoysia/Kentucky Bluegrass/Bahia) lawn age (New (recently laid)/Mature) current condition (Pristine/Great/good/some brown spots) city restrictions on watering (Indicate on a weekly calendar if restricted to certain days; indicate number of days per week can water) enter sprinkler system layout with zones (Allow to upload picture of lawn) zone soil moisture sensor is placed in and how shaded is it.(enter number that and ask whether shaded or not) Do you only want to water your lawn on certain days? (radio buttons for each day want or don’t want to water and include times of the day)
