Create a WordPress Plugin - Secure Form Capture & Export

The project is two-fold:

First, I'd like a form capture plugin that deposits information into a database and saves the information in real-time, field by field. It would be great if it could have the following functionality: to allow a user to create a username & pw, and the ability to return to the form multiple times before final submission. Finally, the ability for the admin to login to Wordpress to view/export the data submitted, as well as the option to have the information emailed or a notification sent to the admin that a form has been submitted (and the admin can login to WP to see the information). [I would need the plugin zip file so that I can re-use it on other projects, as needed.] The plugin would need to work on SSL pages.

Second, I'd like the developer to install the plugin onto a WordPress site and use it for an assessment form which I've created in HTML. It is several pages long, about 24 sections long. It will need to be installed onto the SSL pages of the site.

Please bid for both the plugin creation and the form integration via said plugin into a live site.
