Converting Word Questionnaires to php or .net forms

Hello, I need for this project please an efficient coder who can convert two questionnaire forms (Word doc.) into an online form through php. or .net The original two questionnaires address two different categories of respondents. The needed coder should: 1 **create out of these two questionnaires 1 multiple page questionnaire** with **dynamic content**, i.e. the first page gives the purpose of the questionnaire, personal data of respondents, etc., then upon detecting the respondent's category offers the needed set of questions. 2. **programme the form** to show respondents a **customized "thank you note"** on the browser telling them they will get a "thank you" e-mail for participation and confirmation of their responses... etc. **3. programme the form to send CUSTOMIZED e-mails to respondents upon completion**. In other words, the content of the e-mail sent will differ depending on the category of respondents 4. **programme the form to** generate a **set of follow up e-mails to be sent automatically** later **on specific dates**. It should be noted that we have a **Windows Virtual Server** (VPS), so the coder needs to install database software on it to collect the data of the responses. The programme should aslo be able to export data in Excel sheet. I would like to see **a sample** of the work for approval before carrying on and I would like the coder please to **suggest a user friendly program** to do php programming in case we needed to adjust the forms in the future. Because of previous bad experience, I would like you kindly to state in your response the following, "I will not request any money from the buyer unless he is completely satisfied." Kindest regards. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
