Create and implement 2 web forms on Wordpress platform from .pdf docs for a website (Can use Wordpress plugins)

We have two forms that are downloadable from our website by .pdf. We want to make those forms web forms that can be filled out online and submitted to an email address by email. If there is some current forms Wordpress plugin that will fullfill our functionality and branding requirements we are happy to have you just implement it. Our website is WordPress but NOT a blog. The forms would need to branded to our site. The output would need to be sent by email to two separate addresses. When the submit button is hit the thank you message will direct the form user to a page where they can submit a payment by pay pal. We will provide that code. Further information on the forms is attached to give you a basic idea of what is needed. Please respond with samples of work you have done with forms or indication in your past work with vworker that you have expereince with Web forms and Wordpress. This is third party work. You cannot contact our client in any manner or use this work in any profile or portfolio. ## Deliverables These should be extremely straightforward. were on a Wordpress platform but not a blog. We need 2 webforms created and implemented to send an email with the information entered. Both forms thank you on submit page give a link to a page where they can pay with pay pal buttons. We supply that code. You can use plugins. in fact we have one installed on the site already possibly you could just use it. Here are close examples of the forms. We've take out our clients names.
