A new login page

I am looking for some one to make me a log in page that will look like the file i have attatched. I would like the person to take the source code from the login fields of this page [login to view URL] and then make the login fields of the new log in page look like the one in the attatched file and also work So what we are looking for is some one to make a one page log in page like the one in the file and also make the fields work in the page you make us.
