Create MySQL DB and PHP scripts for form entry

I need a DB in mysql setting up to store form details from a PHP script and be downloadable via CVS files. Firstly I can do this myself, but plan on using freelancers on this and in the future to help manage work load alongside my working projects and time. So your dealing with a technical person which can help and work with you in understanding the requirements and to get good answers any of your questions! The basic setup, There are 4 venues holding an event on one set day each. On this day there are 4 time slots. 6-7, 7-8, 8-9,9-10. During each time slot there are 4 activity types to choose from. Each activity is limited to 150 spaces. Its first come first serve, when places are full the client needs to be informed. The HTMl form. The form will hold spaces for 2 adults and 2 children. And for each participant its their name, address ( if different from first participant), venue, DOB, activity and chosen time. There is then a confirmation step and details are placed into the Database. As stated there are a limited 150 spaces per activity per hour per venue, so this needs to be counted and the user informed. Styling is not an issue, just make sure the form validates entries, keeps data selections on errors in inputs cleans SQL input. To be honest I may even supply you the HTML form and you can simply just write the PHP script and create the DB if preferred. CVS file. There needs to be access to a webpage ( password via php or htaccess if fine) which downloads the latest details for each venue. perhaps [login to view URL] or [login to view URL] etc etc. This must be completed in a less than 2 days from project been taken on. So as stated, I am using this job as a test to see if this website could be a viable option for me in the future.
