create a custom database structure (called NEWDbase) in SQL that updates itself with data from an existing SQL database (lets call it OLDDbase) that runs a dental practice management application. the NewDbase will have only some of the tables from OLDDbase and a whole bunch of new tables that I am looking to keep track of. Then, an application (call it CUSTOMapp) will be written that will look into the NEWDbase to help us book patients into an electronic appointment book. this booking application will run across 3 to 4 computers in the office on a network. This CUSTOMapp may have to be written in ASP but visualbasic would be preferable. This CUSTOMapp will also have several built in features that will involve creating custom reports and some other data extracting tasks. The job is initially budgeted for 50 hours. be prepared to spend 5 hours with me (FACE to FACE is preferred but SKYPE is ok too) studying my needs and then tell me what you can get done for the other 45 hours. Preference will be given to candidates that can meet FACE to FACE in a public setting of course in any major city in USA, CANADA OR EUROPE.

I would prefer web-based, simple reason is you can add as many users through control and no need to install in every user's PC. And on top of the we need to have a windows application which uses this db and make you reports based on the requirement. I have been working on these sort of things from long time. I already have a windows app ready which uses sql db and shown you what all information is required.
