Auto Refresh Appointments Information with out using Timers

We have a Doctor Appointment scheduler windows application (Desktop application) developed in Visual Studio 2010 – VB.NET with SQL Server 2008 as data base. The application is installed on 1000 computers around the world (different LANs (WAN)). The application uses WCF service to communicate with data base server to get and save information.
The secretary’s enters the doctor appointments information from Computer A, Computer C, Computer D, etc…. The doctor is logged into application from Computer B and using the application.
Now we have to auto refresh the appointments information on doctor computer (Computer B) automatically which are given from Computer A, Computer C, Computer D, etc…
Let us know what the best technology to auto refresh appointments information and provide sample application. The sample application should take consideration of below conditions
1. The timers are not allowed
2. The performance is very important
3. We should not put more burden on data base server to auto refresh appointments
