We pride ourselves in reviewing each individual’s care needs from a holistic point of view with exceptional attention to detail.
General Practitioners are supported by an experienced Registered Nurse (RN), who will accompany and assist them in their normal consults, act as a liaison between the residential aged care facility (RACF), GPs and families.
We also provide additional healthcare provisions that include, but not limited to:
• Chronic and acute disease management
• Direct case conferences with allied health input / referrals
• Direct family conferences
• Customised care plans
• Comprehensive medical assessments
• Residential medication management review processes
• Dictation of medical notes
• Direct liaison with specialists
• Detailed handover to RACF staff
Our aim is to construct professional and genuine relationships with RACF’s and work as a team with Allied Health professionals and specialists to provide the best care for each individual.
We also aim to maintain these relationships to alleviate existing nursing staff at RACF’s of their duties to assist General Practitioners during consults.
