Who: pharmacist/ triage focal person d. When: routine, refill, clinic visit 4. For Fast track model, insert the 4 boxes and add the sticky figure of male, female and adolescents: a. When: 3 monthly Refill, 6 monthly clinic visit b. What: ARV + CTX; c. Where: ART clinic d. Who: clinician, Triage focal person, pharmacist 5.
