HTML5 application AOD

I need an HTML5 application developed with the 4 test forms (one sample attached, both PDF and HTM). Will be used on IOS, Android and windows touch screen environment. Choice of IDE / Framework up to the developer. It will collect data offline and later send that data to our web server using https. Application will be off line for 50% of the time (i.e. no internet so need local storage and operation). When online, user taps sync button to push data collected to our web server. Needs to dynamically adjust screen layout for use with tablet and phone format. The App needs home page with a create button for each form type, it will need to show the number of Syncs Available, and needs a "Sync" button.. The screen should also list all tests done on the ipad, showing date/time completed, testees name, and test status: started, completed, synced. There also needs to be a setup page where the User will manually enter “Company ID”, “Company Name”, “User First Name” and “User Last Name”. When closing the setup page, a syncronization must take place. At this stage there is no local data, so the only data returned to the app is the number of syncs available. When a user creates a form, Company ID”, “Company Name”, “User First Name” and “User Last Name” need to be pulled from the apps database to populate the form. Forms need dropdown select for Dates. The signature panels on each form need to be a graphic imput field, allowing the user to use their finger or a stylus to approximate their signature. The app needs to be able to be exited at any time, preserving any data that has been entered, and allowing the form to be continued at a later time - even after a reboot. When one or more tests are complete, the user can tap the Sync button, and the app needs to communicate with our web server and send all the unsynced test data. Once the server has successfully accepted the data it will return a "successful" flag so that the app can change the tests status to "Synced" and decrement the no of available syncs. Synchronizing shoudl start with the oldest test and progress chronologically. A summary screen needs to pop up afterwards, showing the number of tests synced. Users can create tests at any time; if there are more tests stored that the number of available syncs, stop syncing when the number of available syncs =0, and notify the user. Finally, all data stays on device until user select a button to clear data, and need a confirmation screen “Are you sure you want to delete data as it cannot be retrieved”
