Dynamic Interactive PDF Document

I need a PDF specialist to help me do the following: 1 - Replicate the attached file as a generic template - such that each coloured box at the top (buttons) when clicked will open and display some information (Message) and when clicked, the message box will close 2 - I want such that when generating a document from the template, the information displayed in the message box is automatically populated from data saved on a database. This will work like some sort of "mail merge" where by there is a database of values with which each PDF document will be created. I will expect the freelancer to deliver the interactive template as described in 1 above and then a method of automatically generating documents from the template in a way i can manage myself.
