Check Ha Proxy Load Balancer Configuration and Fix Errors

I have setup a HA proxy server on a CENTOS server for load balancing MySQL and IIS web servers on 2 Windows servers.
After the ha proxy was setup everything was fine and was working fine until I had to reload restart the ha proxy several times. Then suddenly, I started getting "The connection was reset" errors from the ha proxy and the database connections were failing. Upon googling up up the error, it appears this has to do with TCP resets from IPtables.
I need someone to check up the ha proxy configuration and provide permanent fix for me. A screenshot of the error is attached.
Please only bid if you have good experience with Ha proxy setup on Linux and MYSQL database ( Windows)
