Simple web survey

We need a survey created as a web based form. There are about 7 questions (example page is in the deliverable section) but they are asked 10 different times for each department so most of your code will be the same. This really is a pretty simple project.I think this is about 4 to 8 hours of work, the deadline will be 3 days so please don’t bid if you don’t plan on working on this immediately. An example screen is attached, this is the basic look and feel 1) Data should be stored in an access database 2) A cookie should be used to recognize the session, If cookies are turned off then a session variable can be used. 3) The survey should be in ASP.NET or ASP (ASP.NET is preferred so please specify which you will use) 4) A progress bar at the top will display their progress through the 10 sections and as they enter questions 5) We will need a small separate page that allows us to download the results in Excel format, this will be in a subfolder of the web 6) If they close their browser and return it should bring them to where they left off, a 5 second status screen should display “Returning to your previous place in the survey?? 7) When they finish the survey if they return they should not be able to take the survey again without confirming it is OK to start again 8) Screen size should not be a fixed width or height but certain items should be oriented to the left or right 9) Navigation buttons will need to be at the top and bottom ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased.
