I manage a website for my hospital department which can be found at [[login to view URL]][1]. As part of our service to referring doctors we have devised a web-based version of a referral protocol for back pain - simply a web form (see [[login to view URL]][2]). Originally the idea was that this could be completed and then submitted electronically to a given email address - but I cannot make that bit work (which is why there is no "submit" button on the page!). However the UK National Health Service has introduced a new outpatient booking system called "Choose and Book" through which an appointment may be arranged, and to which a referral letter may be attached as a file (usually a .doc or .txt file). What our GP colleagues would thus like is the ability to complete the form and then save it with the embedded data, so that they might then send the form as an attachment through the C&B system.
At present it is possible to do this by using "Select all" from the right mouse button, then "Copy" and then Paste into a Word document. However this is somewhat cumbersome and I would like to try and enable the email option. My host site does suggest that any form for submission may be blocked if directed to an email address outside the site system, but I want to make the code work before fiddling with that side!
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
