Android APP for tracking services.

I need an App for tracking services developed with phonegap/*Dreamweaver. This app will be installed for multiple users on Android devices and must meet this requirements. Some users will be acting as client-user requesting services and other users will be acting as employee-user who receive requests and then serve it CAPTURE SERVICE REQUEST (based on a form) (this are some of the few fields) - Identifier of location to serve (is a number from a droplist) - Title for service request (text field) - Short comment (text field) - Capture a picture directly from phone´s camera - Capture date and time (an automatic hidden field) - Capture identity of client-user (may be id-number or name and picture) SEND SERVICE REQUEST (captured previously) - Send when client-user press a confirmation button (to Mysql server online) *preference for sending service request: 1) Wifi, 2) GSM, 3) if no available connection else store form info into a local db (may be Sqlite) - When connection to server be available on user´s device, send data stored on local db, and receive any data pending on server - Send a push notification to Employee-user (those who serve services) REPORT FOR ATTENDED SERVICE (based on a form) When employee-user finish attending a service request (this are some of the few fields) - Identifier of Service Request attended (from a droplist) - Fill up a check list over the Service request. - Short comment (text field) - Capture a picture directly from phone´s camera - Capture date and time (an automatic hidden field) - Capture identity of client-user (may be id-number or name and picture) SEND REPORT OF ATTENDED SERVICE (from requested previously) - Send when employee-user press a confirmation button (to Mysql server) *preference for sending service request: 1) Wifi, 2) GSM, 3) if no available connection else store form info into a local db (may be Sqlite) - When connection to server be available on user´s device, send data stored on local db, and receive any data pending on server - Send a push notification to Client-user (who requested service) SYNCHRONIZED APP Services requests and report of services generated by any user must be synchronized with all users. If one user has no connection when other user send a request or report to server, then this first user must receive from server those requests or reports when a connection be available. Notes: Server DB (MySql) will be provided by me according to developer instructions. The Developer who be selected must provide the complete code of the App. Code provided must include all files for the app, and all files needed on server (front-end and back-end) I realized a resumed presentation in order to show a better idea about what I want for this app. (simulating app screenshots and operation sequence). I´ll send it if candidates ask for it.
