Bootstrap/Paypal Form

I have bootstrap website. I need a form that will email results of the form to me, and collect payment via paypal button. Form styling should be just using standard bootstrap. (I’ll CSS and color it later) Ideally, I would like there to be one submit button and when the user press submit, it would email me the results of the form and have them pay with paypal (predefined amount) in one step. It would be nice if those fields used jquery/bootstrap validation to make sure the email is formatted correctly, phone number, etc.) The form will need these fields: - Name (text field) - Parents Name (text field) - Email Address (email field) - Phone Number (tel input field) - Session Date Requested (text field) - How did you hear about us? (text field) And then on the submit button, it would email me the results of the form and redirect user to pay a predefined amount via paypal. (then after paypal, redirect to a thank you page on my site) (it should be able for me to add a text field to the form later if need be, as well as change the pre-defined $ amount that will be charged by PayPal)
