Capture subscriber IP, location, date and country with HTML Opt in Form in Wordpress / Instabuilder

I need to capture subscriber IP, date, location and country through Instabuilder HTML opt in form installed on wordpress. Instabuilder is just a wordpress plugin that designs landing pages. I dont have any issues capturing the email and name, however I dont know how to get the IP, location, country and submit it with hidden fields in the form. Using php code in value field does not work. I need a robust solution that can be implemented on more than one website. Let me know how you propose to get this done. I attached the html form code. It includes the php code which does not return any values. Likely I need a separate script that would pass these values to the form.
