Complete building of a Form that feeds output for a Directory of group members

This project has been partially completed. We have a Google Form that is filled out by members of our organization with their personal information. That Form populates a Google Sheet that then produces the output in a Google Doc. Please see the attachment which is the current example and has notes that explain the changes needed. The Google Form links are below: [login to view URL] [login to view URL] [login to view URL] There are several areas that must be fixed: The Form has separate entries for First Name and Last Name - those don't populate the Sheet properly. The Sheet needs to automatically alphabetize by last name. The data in Columns 5, 6 and 7 should be listed on new lines or bulleted. The number of rows in the table in the output Doc needs to automatically adjust to accept new entries from the Sheet. The output Doc needs to maintain formatting so that no person's data is split between two pages. The following are areas that would be nice to have, but aren't required: A header with our group name and logo. Show the email addresses all on one line without wrapping, and without making the rest of the columns so narrow that they don't look good either. Column width adjusted so columns 5, 6 and 7 are wide enough so each entry is on one line with no wrapping. Formatting that alternates rows of white and gray or some other way to make the information easy to read. Any other ideas that can make the output look more professional.
