Design 3 Html Pages with minor PHP coding

This is an easy html/php design job. You’ll create a 3 pages. A data input page(only 9 fields) plus name and email, an about-us page and a results page. They should all have the same look and feel. Page-1: Data input page: This page has an introduction that gives people an idea of why they want the report and what info they’ll want to enter. After the text and image formatting It’s just 6-7 questions on a page plus name and email NOTE: Fields should be validated and an error shown in red next to any incomplete or errored field. There should be microhelp for each input field that displays a line of text for each field when clicked. The text to be displayed will be provided soon after the job is awarded. Page-2: Results Page: This page is meant to be really attractive. I’ve added a few graphics to get the ball rolling but I’m actually looking for your design expertise. Once you’re done with this page should not only look visually appealing but it should instinctively convey the information on the page. It’s a table that show how much time we loose on common activities. Page-3: About Us page. Format text and image with a button at the bottom that takes users to Page-1 A php program has been created that takes page-1 input fields and creates results data. Currently, that data is echo’d to the screen but you’ll take that data and populate the results page you design. I've created rough mockup pages. The website will resemble [login to view URL] Page width should be 1000 pixels Header will have height 300 pixels. Should be mobile friendly. Deliverables include and css stylesheet. html pages. Please let me know if additional files are needed to complete the job. Similar font and colors as website mentioned. You must be willing to answer questions about your work. You code must be well-documented and easy to read,. You code must be tested and debugged before delivery. My site uses PHP 5.5.30 Coding should be structured and modular, possibly object oriented and use standard naming conventions. Parameters to functions should be appended with suffix ‘parm’.
