Develop web based survery application - 4 modules

Module 1 : Manage questions in question bank, questions are stored in categories and sub categories with question properties like score, response type etc. Module 2: Create a survey template by slecting questions from the question bank and save the survey for response. Module 3: Gather user response from the survey template collected. The response can be either text, dropdown, radio choice or checkbox choice with additional information if necessary in the form of attachments (multiple files or images), Comments, display progress and input paramaters like interviewers name etc. Module 4: Generate basic chart and data output, calculate basic scores etc. This is phase one of the application and no user login or authentication is needed for now. The admin template will be supplied to make essential pages and forms. Phase two will be seperate project with enhancements and additional budget. I have a desktop application simillar; of the developer wants I can show a onscreen demo of existing application using team viewer. I expect this application to be web based with responsive on Tablet (no mobile phone). Maximum budget is SGD 800 and expect complete development before 8th Jan 2018. Ideal platform or technologies will be PHP, MYSQL, MONGODB, JAVASCRIPT Excellent communication skills, flexibility, availability, are key elements for selection. No advance release. Can milestone and release 40% upon completion for first 2 modules. and rest upon completion of rest.
