I believe the generate PDF will have the information in text form instead of corresponding form controls (as present in the HTML template). There are two initial questions: 1. How many fields/pages do each of these templates contain? 2. If a template runs across multiple pages, should headers and footers appear on each page, or just the first and the last page?
