Modify MRTG Index Page w/ Checkboxes

Need to create javascript include that will modify a basic HTML page with checkboxes next to items. Once submitted all items will get posted to another url to be added to a database to create a new index page.

Must know MRTG and ASP.NET and SQL. Needs checkbox selections to be all saved to a table in a sql database.

Each entry on these index pages have a standard template with this header:

I need to have the ability to include a single javascript reference into existing HTML page. This adds a "SELECT" button (floating, at top, bottom, etc) that when pushed adds checkboxes to every single entry on the page. The SELECT button then should change to "SAVE" button that when pushes takes all selected checkboxes and publishes them to another asp.net page that basically asks for a title of that group and submits them all to a sql database table. I can then use this database to create new index files for users.
