I will clarify my needs here with various examples in order to be accurate as possible
I created 4 forms on one responsive page on my sub-domain protected page.
These forms are for my own purposes of database entry in my back-end.
Each form has around 15 to 20 fields, that need to be created in the database.
My needs are in MySQLi:
1. To get each row of the database, with pixel perfect layout in PDF for printing, when it’s been called from the front-end, or back-end.
2. I will also need a "search box" PHP code to put on my front-end. This search box function will find the specific row fields details that I entered in the past from my back-end.
The search will be based on specific column with 8 digits in a designated column in my database called "id_cert".
To illustrate: let’s say a visitor input an 8 digits number on front end search box, a php script looks at my database, and importing those specific row fields into a pdf ready to print for my visitor.
3. creating a database reader on my back-end (importing from the tables and rows in the database).
4. This PDF, should also be available on the back-end (through the database reader), so I will be able to "print" "view" and "edit", after submitting the data into the database with my form.
5. The PDF need to have the same background image covering the full page (11x8.5) on all 4 forms. Also, each PDF will have the row fields, and another unique image from the database (can be a URL if you decide it’s better to upload the image from the form into a folder on my site, and just use the URL’s as a reference), on top of the background image.
6. On my back-end, the PDF doesn't need the background image, as I PRE-printed the background image on A4 pages already. So when ill click "print" in the back-end, it will just print all the fields and image layout. But on the front end, my visitors need to see the background image too.
And now in even simpler words:
1. Search box script to put on my front-end site, where visitors need to input 8 digit numbers. This 8 digits number is associated to every form (each form is a row in my tables).
2. The ability for a PDF to be generated and showed to a visitor as the 8 digit number finds a match in the database.
3. The ability for me to have a reader of my database fields on a page in my back-end, with sorting capabilities by clicking the column names.
4. Adding columns to this reader, such as "Print", "View" and "Edit" to each row.
5. Pixel perfect layout of the PDF's by my specification. (I got a Photoshop PSD or JPG for clarity).
You can use FPDF, TCPDF, PDFlib or anything you like, as long its fast and working on mobile too.
I think about creating 4 tables for this, as I have 4 forms, but I would like to get all tables view under my database reader, based on date of creation.
Please view my images for clarity. These images are "just for illustration", as our forms have around 15 to 20 fields. Its just an idea for the functions we need.
1. “search-box” image, shows we need a box with a PHP code calling the database
2. “A4 pages we have” image shows our printed pages we already have.
3. “our print needs” image, shows what we need to print, when we click print in the backend.
4. “PDF visitor will see” image, is what we need to be in the pdf when a visitor search the box, and then get a PDF open on his screen.
5. “database-reader” image, is what we need on our backend.

1) 4 forms coded to PHP/MYSQL backend with errors
2) Admin panel to show results of datas when these forms filled up
3) Unique id_cert created and search form on both backend and frontend will be coded.
4) PDF page like you need will be coded too
All these i need the price i had bid for. If ok lets discuss more on this. i dont do html/css photoshop work. I can do php/MYSQL programming in it.
