Need a PHP Laravel developer for an on going HRM project.

I have a HRM project which needs to be done based on clients requirements, its 50% completed and has 50% requirement to be done. Before I hire I will assign a small task to test the freelancer skills its small task. The task is mentioned below : we need to add some changes to it like VISA feature, below link has the form page which are the fields required, you can refer them to design the form. Leave the payment feature mentioned in the form, we don’t need that as of now, only the basic information would do. So you can avoid any kind of payment feature. Admin should have an option to set a limit as when the alert should be given before 1 month or 2 month or 3 month user should be able to set that. The budget I have set is only for this task, this is a long term project. So final budget will be discussed after the task is done.
