Design a form and customize its submission

I need a form designed to submit data values. the form will have 4 fields that are not repeating name email address phone after these fields are filled in there will be 8 fields that will be required to be filled in. These 8 fields can be repeated 100 times. when the form is submitted i need the repeated fields to be emailed in columns and not seperate row for each field. I have already designed a form like this, but nobody can help me with the plugin for the submission requirement, maybe it will be easier to design the form and customise the submission one time
