My proposal is that we add a Service Reference to the web page/app and start from there. If we needed, then use of Microsoft's SVCUtil to generate VB code from just the URL or WSDL can also give us the contract code needed. I see this doable within half a day's work. We will need to discuss details on how you want the form to look and what fields are needed.
